This is a website notice. It is not the office Notice of Privacy Practices for medical records. If you have a question about your chart, your bill, or how the office uses health information, call the office.

What this website does not collect
This website has no patient accounts and no form that sends answers to the office. Visits are arranged by phone.
Download the paperwork opens a PDF on your device or in a new tab. On a phone, the Print button opens the same PDF so you can print or save it. You fill out the packet on your device or on paper. This website does not upload, email, or store what you write, including a Social Security number, an insurance ID, medications, health history, or a signature.
Do not email health details, insurance ID numbers, or Social Security numbers to footspecialist@myyahoo.com. That address is ordinary email. It is not a secure patient portal. Call the office when you need to share health information.
What this website remembers
Your language choice is saved in this browser. It stays on your device so the site can open in the same language next time. The office does not receive it. You can remove it by clearing the saved data for this website in your browser, or switch the language with the English / Español control.
Calls, email, and fax
A phone link opens your phone. An email link opens your email app. The fax number is only printed on the page. Those messages go to the office. This website does not store them.
Other services on the page
The typefaces load from Google Fonts. The Visit section on the home page embeds a map from Google Maps. When your browser asks Google for the fonts or the map, Google can receive your IP address and ordinary technical details, such as the browser you use. Read the Google privacy policy.
Directions, on the home page, also opens Google Maps. Once that request reaches Google, the Google privacy policy applies.
This website does not use advertising cookies, analytics tools, or tracking pixels. It does not sell personal information.
If the site is on the public web, the company that hosts it may keep ordinary connection logs, such as an IP address, the browser, the page requested, and the date and time. Those logs are part of running the site. The public copy of this site is hosted on Netlify.
Children
This website is for adults looking for foot and ankle care. It is not directed to children under 13, and it does not knowingly collect personal information from children through the site.
Medical records
Health information you give the office by phone, fax, mail, or in person is handled by the office, not by this website. The new-patient paperwork includes a privacy acknowledgement you can read before you sign it at the office. Ask the office if you want a copy of its privacy practices for medical records.
Changes
If this notice changes, the date at the top of this page will change with it.
